INTRODUCTION Food is vital to life and well-being. It may be defined as any solid or liquid substance which when consumed provides the necessary materials or nutrients to enable the body to grow, replace worn-out and damaged parts and to function optimally. The intimate life sustaining relationship between food and man is called nutrition.


One way of ensuring that health and fitness are maintained is to have an understanding of food and its benefits and to use this knowledge to prevent common life style diseases such as diabetes. It affects 150 million people worldwide and this number is predicted to double by the year 2025. Diabetes is a preventable or controllable disorder but if allowed to go unabated and untreated can contribute to premature deaths, blindness, infections, amputations and loss of valuable human potential.


CAUSES OF DIABETES § Not confirmed. § May be hereditary and/or environmental. § Viruses or obesity may precipitate the disease.

TYPES OF DIABETES There are basically two types of diabetes: type 1 and type 2.


TYPE 1 Type 1 diabetes or insulin-dependent diabetes mellitus (IDDM): § Occurs between the ages 1-40. § Accounts for 10-20 % of all diabetic cases. § Characterized by little insulin being produced. § Rapid onset. § Requires insulin injections and carefully controlled diet. § Sometimes this form of diabetes may be difficult to treat.


TYPE 2 Type 2 diabetes or non-insulin-dependent diabetes mellitus (NIDDM): § Occurs between the ages 40 and over. § Accounts for 80-90 % of most cases of diabetes worldwide. § Type 2 diabetes develops when the production of insulin is insufficient to overcome the underlying abnormality of increased resistance to its action. The early stages of type 2 diabetes are characterized by overproduction of insulin.


As the disease progresses, process insulin levels may fall as a result of partial failure of the insulin producing beta cells of the pancreas. § Chronic onset. § Requires controlled diet, exercise, or oral glucose lowering medication. SIGNS & SYMPTOMS OF DIABETES § Extreme thirst. § Poor concentration. § Frequent urination. § Abnormal concentrations of glucose in blood. § Wasting of body tissue, causing a marked reduction in body weight. § Weakness. § Fatigue. § Nerve damage. § Retina degeneration. § Arteriosclerosis. § Ketosis. § Retina degeneration. § Blindness. § Nerve degeneration and lack of sensation in arms, legs and other body parts. § Infections of the urinary tract; kidneys, bladder, ureter and urethra.


COMPLICATIONS OF DIABETES Complications of diabetes include: § Blindness. § Kidney failure. § Foot ulceration, which may lead to gangrene and subsequent amputation. § Increased risk of infections. § Coronary heart disease and stroke. § The enormous and escalating economic and social costs of type 2 diabetes.


WORLDWIDE TRENDS AND ADVANCES IN DIABETES § The prevalence and incidence of diabetes have increased globally in both developing and developed countries transition. § Worldwide, the number of cases of diabetes is currently estimated to be around 150 million. This number is predicted to double by 2025, with the greatest number of cases being expected in China and India.


These numbers may represent an underestimate and there are likely to be many undiagnosed cases. § Previously a disease of the middle-aged and elderly, diabetes has recently escalated in all age groups and is now being identified in younger and younger age groups, including adolescents and children, especially in high-risk populations. § In Caucasian populations, much of the excess mortality as a result of diabetes is attributable to cardiovascular disease, especially coronary heart disease; amongst Asian and


American Indian populations, renal disease is a major contributor, whereas in some developing nations, infections are an important cause of death. § In all societies where overweight and obesity are prevalent there may be an increased risk of diabetes. § Increased physical activity reduces the risk of developing diabetes.


SPECIFIC RECOMMENDATIONS FOR MANAGEMENT & TREATMENT OF DIABETES Measures aimed at reducing overweight and obesity, and cardiovascular disease are likely to also reduce the risk of developing diabetes and its complications. Lifestyle modification is pivotal in both treatment and attempts to prevent diabetes.


Some measures are particularly relevant to reducing the risk for diabetes; these are listed below:

§ Adopting an exchange diet lists, for example using the lists developed by the American Diabetes Association and American Dietetic Association lists food with interchangeable nutrients and kcal contents used in specific forms of diet therapy. Using the exchange lists, food categorized by type contains approximately equal amounts of kcal, carbohydrates, proteins and fats.


This means that one food type can be substituted for any other food types and still provide the patient with the prescribed types and amounts of nutrients and kcal. § Prevention/treatment of overweight and obesity, particularly in highrisk groups through educational and selective feeding programmes. § Improving the nutritional status of young children. § Promoting linear growth and preventing energy excess by limiting intake of energy-dense foods, controlling the quality of fat supply, and facilitating physical activity. §


Maintaining an optimum Body Mass Index (BMI), i.e. at the lower end of the normal range. For the adult population, this means maintaining a mean BMI in the range 21-23 kg/m2 and avoiding weight gain (>5 kg) in adult life. § Practicing an endurance activity at moderate or greater level of intensity (e.g. brisk walking) for one hour or more per day on most days per week. § Ensuring that the dietary intake of saturated fat does not exceed 10 % of total energy and for high-risk groups, fat intake should be < 7 % of total energy. § Achieving adequate intake of wholegrain cereals, legumes, fruits and vegetables.


A minimum daily intake of 20 g is recommended. CONCLUSION Providing optimum nourishment in the diet for patients may prevent symptoms and delay complications of diabetes. Treatment of diabetes must be individually tailored to the patient needs and lifestyle. Diet alone or by a diet combined with insulin or glucose lowering medication cannot achieve maximum benefits. The treatment regime must include regulated exercise and regular monitoring of the blood glucose levels. It is important to reassure the diabetic patient that one can live a near-normal life if the diet is followed, medication is taken as prescribed and sufficient time is allowed for rest and exercise. Meals should be eaten at regular intervals. Additionally, adjustments should be made to shopping, cooking and eating habits.

Guidance on the Proper Nutrition for Teens

Although it is a difficult task to watch over the health of your teenage child, it is an important time of growth and development for him or her. As your teenager moves into adulthood, his or her body goes through rapid changes, many with permanent effects. It is important to ensure that your teenager is getting the right nutritional balance each day. The best way to do this is to provide your teenager with the resources that he or she needs in order to know the consequences and benefits of dietary choices. In doing this, you can help your teenager to want to eat the right type of meals each day.


Vigorous exercise (i.e. training to an intensity of 80-90% of age-predicted maximum heart rate for at least 20 minutes, at least five times per week) has the potential to substantially enhance insulin sensitivity. § Higher consumption of saturated fatty acids has been associated with higher fasting insulin, lower insulin sensitivity and a higher risk of diabetes. § Higher consumption of unsaturated fatty acids from vegetable sources and polyunsaturated fatty acids has been associated with a reduced risk of diabetes and lower fasting and 2-hour glucose concentrations. § Higher consumption of wholegrain cereals, vegetables and fruits in the diet has been associated with a reduced risk of progression of impaired glucose tolerance.


Issues concerning teenagers today

Of course when dealing with teenagers on health or any other topic, you have to recognize the powerful influence that their peers have and will have on them.
