For the past month I have been planning my long-distance bike ride across the United States and in doing so I have been considering challenging routes, as I do not want it to be too easy. The harder the better for me; sure most people think it would be great to just finish and ride 50-70 miles per day with the occasional 100-mile ride and rest one day per week, not me. You see I want to ride 150-250 miles per day and take two shifts of 15-25 hours per day.

Strike a healthy balance between activity and rest. Remember throughout the day to plan little breaks and relax, relax, relax.


Sweat It Out! Sweat is one way the body cleanses itself. Impurities like alcohol and nicotine along with excess sodium can be eliminated through perspiration. You can sweat some other harmful heavy metals that can cause serious health problems if allowed to accumulate in the body. Exercise, warm baths or piles of blankets will help you to sweat. Exercise Duration You need to perform at lease 30 minutes of moderate intensity activities 5 to 6 days a week. You will increase cardiovascular endurance, overall endurance, general organ strength, muscle strength, flexibility, raise your metabolism, reduce body fat and lower risk factors for hypertension, adult onset diabetes, heart disease and osteoporosis. Of course, you must have a healthy diet to gain these benefits.


Choose an exercise routine that will raise your heart rate to your target zone. Stay in your target zone for 20 minutes. Your cool down should last for 5 minutes. You will burn body fat, lower your cholesterol, blood pressure and risk factors for degenerative diseases. Keep your heart rate at 50 to 60% of your target zone at first, increasing the intensity of your work-out as your fitness level improves. Exercise intensity is determined by your fitness level. Light intensity activities are slow walking, easy cycling, gardening and most household activities which raise the heart rate 50 to 60% of your maximum heart rate. Moderate activities are fast walking, jogging, past paced cycling, competitive swimming and aerobic dancing which raise the heart rate 60% to 70% of your maximum heart rate. High intensity activities are weight lifting, sprinting and mountain climbing which raise the heart rate to 70% to 80% of your maximum heart rate.

Prostate Treatments: Therapeutic Options Based on Cancer Stage


It is important for men to know their options when it comes to prostate treatments specific to a cancer's stage. Staging is used to describe the extent by which a cancer has spread. In prostate cancer, the most commonly used staging system is called the TNM. The overall stage under the TNM system is expressed in Roman numerals. The least advanced condition is tagged as Stage I, while the most advanced is categorized as Stage IV.


In considering prostate treatments, doctors usually look at the stage of the patient's cancer and his Gleason score and prostate specific antigen level results. Other factors such as age, life expectancy, other disorders suffered from by the patient (if any) and risk of cancer recurrence after treatment are also important factors in choosing treatment options.


Cancers that are categorized as Stage I are usually small and have low Gleason scores. They grow slowly and may not produce any symptom or create health problems. For older men who have cancer categorized as Stage I, watchful waiting or radiation therapy are the most common options. These treatment methods are also recommended to those who have other serious health problems. For men who are younger and have no other serious disorders, watchful waiting, radical prostatectomy and radiation therapy are viable options.


LRP, on the other hand, involves the use of several smaller incisions and specialized instruments. This highly complex procedure is known for its high precision and control. In the hands of experienced surgeons, it becomes an advantageous option compared with retropubic and perineal prostatectomy. TURP, meanwhile, makes use of an instrument called a resectoscope which is passed through the end of the penis into the urethra at the level of the prostate. The electricity that passes through the instrument cuts or vaporizes the issue in the prostate. TURP is done to relieve symptoms and is also used for benign prostatic hyperplasia.


Compared with Stage I prostate cancers, Stage II usually manifest symptoms and has a higher possibility of spreading beyond the prostate. For elderly men and for those who suffer from other serious diseases, watchful waiting is the best option, although radical prostatectomy and radiation therapy can also be considered. For younger, healthy men with Stage II cancer, treatment options include radical prostatectomy, which could involve the removal of the pelvic lymph nodes and is sometimes preceded by hormone therapy; external radiation; brachytherapy; combined brachytherapy and external beam radiation; and radical prostatectomy to be followed by external beam radiation if the patient has a high Gleason score. In Stage II, all radiation options may be accompanied by three to six months of hormone therapy.


Stage III cancers are those that have spread beyond the prostate gland but have not reached the bladder, rectum, lymph nodes or distant organs. Treatment options for Stage III include external beam radiation plus hormone therapy; hormone therapy only; radical prostatectomy in selected cases which may be followed by radiation therapy; and watchful waiting for older men and those suffering from other illnesses and whose cancer is causing no symptoms. Surgery and radiation therapy may still be used at this stage but there is a possibility that they might not work.


Stage IV cancers, or those that have spread to the bladder, rectum, lymph nodes, bones and other distant organs, are generally considered as non-curable. Treatment options can still be used, though, and they can include hormone therapy; external beam radiation plus hormone therapy; surgery; watchful waiting for older men; and chemotherapy. Prostate treatments for cancer vary depending on the cancer's stage. The Best Prostate Cancer Treatment for Each Type of Patient

Whether a therapeutic technique is the best prostate cancer treatment for a patient depends on various factors. In selecting the treatment, a patient and his doctor should take into consideration the patient's age and expected life span, the stage and grade of the cancer, possible side effects and other health problems that the patient might have. One of the best prostate cancer treatment techniques, particularly for older men and those who suffer from other serious illnesses, is the expectant management or watchful waiting method. Watchful waiting involves the close monitoring of the cancer through prostate specific antigen testing. It does not involve active treatments like surgery and radiation therapy and is recommended mostly to those who have no symptoms. It is also used when the cancer is contained within one area of the prostate gland and is expected to grow very slowly.


Another option available to prostate cancer patients is surgery. This can be radical retropubic prostatectomy, radical perineal prostatectomy, laparoscopic radical prostatectomy (LRP) or transurethral resection of the prostate (TURP). In retropubic prostatectomy, the surgeon makes an incision in the lower abdomen to remove the prostate gland. Lymph nodes around the prostate might also be removed depending on whether the cancer has spread to these parts. In perineal prostatectomy, the incision is made in the perineum or the skin between anus and scrotum. This procedure is use less often because the lymph nodes cannot be removed and there is a high probability that the nerves will be affected.


Radiation therapy is the method in which high-energy rays or particles are used to kill cancer cells. This, like surgical procedures, is another example of a highly developed method of dealing with cancer. Radiation therapy is classified into two main types, the external beam radiation therapy (ERBT) and brachytherapy. In treating localized prostate cancer, a procedure called crysosurgery is sometimes used. It involves the freezing of the tissues using very cold gases. Aside from cryosurgery, hormone therapy and chemotherapy are also options that prostate cancer patients can explore. Hormone therapy does not cure cancer but is primarily used to lower levels of male hormones in a patient's body. Chemotherapy, meanwhile, is a procedure more commonly used in cases when the cancer has spread beyond the prostate gland.
