Lung Cancer - SURGERY As A Treatment Option


For some lung cancers in the early stages, an option that offers the best chance for a complete recovery is surgery. Medical literature has shown this to be true when a patient is diagnosed with non-small cell cancer and the detection is made in the early stages and the tumor is small and no evidence is present that it has spread to any other organ systems, if surgery is used to remove the tumor, the patient has up to an 80% chance of passing the five year survival mark.


The reason for this is because most small cell is seldom diagnosed before it's already begun to spread to other parts of the body.


Surgery is recommended for some but not others - why? Surgical treatment is normally recommended for non-small cell cancers, especially in the early stages. Surgery MAY be recommended for small cell IF it's diagnosed in the earliest stages. On the other hand, non-small cell does not spread as fast or as virulently. Non-small cell tends to localize into one spot, therefore, there is a greater chance that surgery can remove all the cancer in one surgery.


Depending on where the tumor is located is how the decision is formed as to whether or not surgery is appropriate. If it is located too close to the heart, the windpipe, major blood vessels or other major organs, the risks and dangers are much greater if surgery is attempted. In these cases your doctor may recommend radiation or chemotherapy to shrink and kill the cancerous cells. The different kinds of surgery for treatment. Depending on the location and size of the tumor found, there are three main types of surgery that are used.


If it has been found in it's early stage and is confined to a very small portion of the lung, an oncologist may perform a 'lung resection' or a 'segmentectomy'. During each of those surgeries the doctor removes a small section of one lung, the area where the cancerous cells have been detected. If it's determined that it may have invaded the nearby cells though, the doctor will then use more radical surgeries. The removal of one or more lobes of the lung, but not the entire organ, is called a 'lobectomy'. If the thoracic surgeon believes that only part of it is affected and that it has not spread to the entire organ, the surgeon will opt for a lobectomy.


However, if it is believed the entire lung may be involved, he may then choose to do a pneumonectomy. A 'pneumonectomy' is the removal of the entire lung. Prior to this surgery, the doctor will perform tests to be sure that the remaining lung will be capable of supporting your needs for oxygen before doing a complete pneumonectomy.


Surgery is a very invasive treatment and is not the treatment of choice for most cancers. Doctors will only attempt surgery if there is a chance that it will remove it in its entirety. If it has advanced beyond a small portion, or it's fast spreading, then surgery to remove it does not make any sense. In these instances, radiology, chemotherapy or some other type of treatment would be a more reasonable alternative.

Breast Cancer And Pregnancy


Although it is rare for breast cancer to strike younger women, the fact remains that all women are at risk. And for those of childbearing age, the first sign and symptoms of breast cancer leading to a diagnosis can not only be upsetting and unexpected, but complicated as well. Developing breast cancer at a younger age--in a woman's 40s, 30s, even 20s--will mean making important and difficult decisions about one's life and future perhaps much sooner than originally expected.

One concern is developing breast cancer during pregnancy, which although rare, can still occur. In this case, the treatment chosen will not only affect the patient and her body, but the growing baby inside her as well.

It will depend on what stage of pregnancy she is in (first, second or third trimester) and what stage her cancer is in--such as whether or not it's advanced. Most pregnant women can have treatment for their breast cancer without affecting the baby. But some might be advised by their obstetrician or health-care practitioner--or even decide themselves--to terminate the pregnancy, more so if the pregnancy is in its earlier stages, in order to receive certain treatments that would be too risky otherwise. But it is essential to remember that it is a woman's own decision--it is not medically necessary to terminate a pregnancy if the expectant mother is diagnosed with breast cancer. All it does is limit treatment options. Breast cancer itself will not affect the fetus--only certain tests and treatments will.


Generally speaking, tamoxifen, chemotherapy, radiation, and other drug-related therapies are avoided if the woman is pregnant because of their associated risks with birth defects. Tamoxifen, especially, is considered very unsafe because it is a hormonal therapy and is never recommended if the woman is pregnant or planning on conceiving. Surgery--either a lumpectomy or mastectomy--is the most common and preferred method of treatment for breast cancer in pregnant women.


Another concern is whether or not breast cancer survivors can or should go on to have children after treatment and recovery. It's a very controversial issue with firm advocates on both sides of the debate. There are two main questions here, for both the medical and health community and breast cancer survivors wanting their own children:


1) Do certain breast cancer treatments affect fertility?; and

2) Is it actually considered safe to conceive and carry a baby to term following breast cancer and breast cancer treatments? As far as fertility goes, there is no definite answer here. For chemotherapy, it depends on the age and what specific drug was used--some affect fertility more than others. And taking tamoxifen after chemotherapy to prevent recurrence is not recommended if the woman desires to become pregnant right away. Although tamoxifen is sometimes used as a fertility treatment, there is evidence to suggest that it damages developing embryos, and therefore is not considered safe to use.


Many doctors caution these women to wait several years to ensure receiving the best breast cancer treatment possible and to go past the point of the biggest threat of breast cancer recurrence. But some women decide to go ahead and have babies anyway, since it's so important to them.

Risk Factors For Prostate Cancer

The prostate is a small, walnut-shaped sex gland in men that produces the seminal fluid, which nourishes and transports sperm. The growth of the cells in the prostate gland is stimulated by the male sex hormone called testosterone. Though its causes are unknown, prostate cancer is a frightening prospect for men. This cancer threatens not just their lives, but also their sexuality. Possible consequences of treatment (even if the treatment has been successful in saving a person's life) include erectile dysfunction and bladder control problems. Prostate cancer progresses very slowly and the early stages show little or no symptoms. If detected early, effective treatment with minimum side effects is possible. Once the cancer spreads (metastizes) treatment becomes more difficult.

A man's vulnerability to prostate cancer increases with age. Most often, prostate cancer is detected very late and people who lose their lives do not die from prostate cancer, but die WITH prostate cancer. As the cancer develops, it eventually squeezes the urethra, which surrounds the prostate.


This is when signs and symptoms begin to appear:

-Urgency in urination -Difficulty in starting urination -Dull, persistent ache in the lower pelvic area -Painful urination, a very slow flow (almost like a dribble) -Intermittent urine flow, and a sensation that the bladder is not empty -Frequent urination, sometimes including blood -Painful ejaculation -Persistent pain in the bones, lower back, hip and thighs -General ill health, loss of appetite, and decline in weight If the cells from the cancerous prostrate break away, the cancer will spread. Most commonly, prostate cancer spreads to the lymph nodes, bones in the hips or the lower back, lungs, and sometimes even the brain.


Risk factors: The risk of prostate cancer increases with age. As with almost any other cancer, heredity also plays a key role. For reasons unknown, African-American men seem to have a higher risk of prostate cancer. A fat-rich diet and sedentary lifestyle also contributes to the risk. Fat causes an increased production of testosterone, which may in turn lead to the development of cancer cells. High levels of testosterone mean higher chances of developing prostate cancer. Therefore men who suffer from hypogonadism, or have been undergoing long-term testosterone treatments are at risk. A vasectomy may also result in prostate cancer, though there is no conclusive proof of this.


A routine screening test may reveal the beginnings of prostate cancer. A DRE (Digital Rectal Examination), which involves inserting a gloved finger into the rectum, helps the doctor to examine the prostate. Any change in shape or size of the gland may mean trouble. A prostate-specific antigen (PSA) test analyzes a blood sample for the levels of PSA. If a higher than normal level is detected, a prostate infection or cancer may be suspected. A transrectal ultrasound helps to further evaluate the prostate using sound waves. If initial tests produce positive results, a prostate biopsy may be done to verify the presence of cancer. If cancer is confirmed, the next step is to investigate the spread of the cancer. A bone scan, CT scan, and Ultrasound scan may be used for this. The mode of treatment directly depends on how aggressive the cancer is. For most men, a combination of treatments (surgery, radiation paired with hormone therapy, chemotherapy) may prove effective.


The best way to reduce the risk from prostate cancer is to eat a diet rich in fiber and low in fat. Regular exercise also helps.

Breast cancer usually happens when certain cells located in the breast start to grow out of control, taking over nearby tissue and spreading throughout the body. Large collections of this infected tissue are called "tumors". Some tumors are not even considered to be cancer because they cannot spread throughout the body or threaten a person's life. These types of tumors are called "benign tumors". The types of tumors that do spread throughout the body and invade the tissues around the breast are considered to be cancer and have been given the name "malignant tumors". It is said that any type of tissue in the breast can form some type of cancer, but it mostly comes from either ducts or glands.


It can take months or even years for a tumor to get big enough for someone to actually feel it in their breast, so they are encouraged to be screened for tumors by a mammogram. Mammograms are designed to detect any type of disease before a person even begins to feel it. Breast cancer is the most common "malignancy" that affects women in America and throughout Europe. Every single woman is at risk of getting breast cancer and almost 200,000 cases of were said to be diagnosed in the United States in 2001. It is the second highest cause, behind lung cancer, of cancer deaths among women in North America.


The types of risk factors for breast cancer are divided into two types, those you can't change and those you can change. The factors associated with increasing your risk of breast cancer that your can't change include: just being a woman, getting older, or having some type of family history or a relative with breast cancer. Other risk factors are having your menopause late, having children past the age of thirty, or contracting a genetic mutation that would somehow increase your risk.


Certain types of risk factors that you can change are:

- not taking hormone replacement therapy (HRT), which means that long term uses of estrogens for menopause symptoms does in fact slightly increase your risk.

- stopping the use of birth control pills, because it is noted that ten years following the cessation of The Pill, a woman's risk of breast cancer reverts to what it would have been if she had not used this form of birth control.


However, neither of these risks are as significant as the ones that are associated with your gender, age, and family history. All the factors are based on probabilities, and that means that a person without any of those mentioned is still subject to developing breast cancer and the best way to be sure is to get the proper screening and detection for breast cancer development. There are preventative measures that can be taken, up to a point. It is noted that a drug called Tamoxifen is not used widely as a prevention, but it has been proved to be helpful in some cases. There is also a limited amount of data that suggests that Vitamin A may be useful in the prevention of breast cancer, but further research is needed to prove this.


The most important step for a woman to prevent breast cancer is to schedule regular checkups, screenings and mammograms, learn how to perform her own exams, and also to acquire all the information she can about the subject.
