Juvenile Arthritis (part 2) Polyarticular and Systemic Juvenile Arthritis

What is Polyarticular Juvenile Arthritis?

Polyarticular Juvenile Arthritis is a type of arthritis that affects children between twelve and sixteen years of age. This disease affects roughly one third of Juvenile arthritis sufferers and mainly targets females. The joints affected are usually evenly distributed between the right and left sides of the body.

Systemic Juvenile Arthritis is a more serious form of the disease and it is this form that is most likely to continue into adulthood.

For instance if the right foot is affected, the left foot is typically also affected. The joints affected in polyarticular juvenile arthritis are usually the smaller joints for instance the finger joints, however in some cases the larger joints such as the knee or hip joint are also involved. This form of juvenile arthritis also involves more joints than the more common pauciarticular form, at least five or more joints can be swollen and painful.A few children with polyarticular juvenile arthritis may have the rheumatoid factor in their blood (RH) which means the disease can manifest as rheumatoid arthritis later in life, but fortunately most will grow out of it.

 

Systemic juvenile arthritis can also affect the lymph system and internal organs, such as the heart and liver. On the other hand, few children with the systemic form carry the RH factor which can develop into rheumatoid arthritis.

 

Arthritis in children can vary from day to day. Make the most of the days that your child is feeling better. It's important that they get enough exercise to build their muscles and ensure healthy growth.Meanwhile research continues into this disease on all levels. How to Treat Rheumatoid Arthritis?

 

Traditionally, non-steroidal anti-inflammatory drugs (NSAIDs) go as the first-line therapy. Aspirin, ibuprofen, naproxen, sulindac, indomethacin, nabumetone, tolmetin relieve symptoms. However, NSAIDs can not prevent joint destruction. Corticosteroids efficiently reduce the symptoms of inflammation. Alas, the side effects of corticosteroids are extremely extensive. They cause weight gain, weak bones, infection, acne, easy bruising, diabetes, facial puffiness, stomach ulcers, eye cataracts and many others.

 

Gold compounds and anti-malaria drugs are also in use.

 

In case of active rheumatoid arthritis the disease-modifying anti-rheumatic drugs (DMARDs) are used. An example is methotrexate. Doctors often use DMARDS together with corticosteroids and NSAIDs. Other drugs include: azathioprine, cyclosporine A, D-penicillamine, minocycline, sulfasalazine. Majority of these drugs have serious side effects and complications. Currently DMARDs are not well tolerated and have serious side-effects. Corticosteroids are still widely used, but doctors expect development of newer and better DMARDs. Opiates and topical lidocaine relieve pain.

 

Rheumatoid arthritis cause extra production of pro-inflammatory cytokines (the regulating substances of immunological system) and relative lack of anti-inflammatory cytokines. This imbalance leads to the development of cartilage and bone destruction. The process erodes bone in the joints. Researchers found biologic agents to block some specific cytokines. For, example anti-TNF monoclonal antibodies infliximab and adalimumab block TNF (Tumor Necrosis Factor, one of the most potent cytokines). Another strategy use etanercept (a soluble TNF receptor) that reacts with TNF and prevents attachment of TNF to the real TNF receptors on the target cells. An IL-1 receptor antagonist anakinra prevents another type of inflammatory response. Use of these biological agents made significant and rapid clinical improvements with low toxicity.

 

Alas, there were some drawbacks after long-term use. One was the need for parenteral administration (needlesticks) which is unpleasant and may lead to infections. Another was that the agents cause an immune reaction of the body and loose efficiency over time. Furthermore, monoclonal antibodies and soluble receptors inhibit on the extracellular level. More effective would be to block intracellular inflammatory pathways. Inhibition of p38 MAPK by oral drugs is the possibility under investigation at present.Weight loss, physiotherapy, joint injections, and specialized tools for home activity help the patients. In severe case patient may need joint replacement. Some patients benefit form changes of diet and lifestyle. Reducing stress, sleeping well, eating less and highly digestible foods, eating yogurt, garlic, vegetables, drinking sufficient volume of water may help some patients.

What is Rheumatoid Arthritis?

Rheumatoid arthritis is the destructive disease that targets primarily joints. However, it also affects many other organs and systems, including brain, heart, kidneys, spleen, blood vessels, lungs, and muscles.

Rheumatoid arthritis affects around 1 in 100 people.

In developed countries rheumatoid arthritis is the most common autoimmune disease. Though rheumatoid arthritis may start at any age, it is more common after sixty. One in twenty women over the age of 65 years suffers the disease. Economically it is very expensive disease. Besides high direct medical cost, there is much greater indirect medical price tag. Rheumatoid arthritis cause many complications, requires drastic therapies and orthopedic surgery. Doctors estimate that rheumatoid arthritis makes a one in-three patient permanently disabled. Half of the patients stop work in first decade of diagnosis.

 

Rheumatoid arthritis is a chronic disease. It leads to the loss of mobility due to joint destruction and pain. The joints suffer in a polyarticular manner. That means that many joints affected at the same time. Usually in the beginning joint problems are asymmetrical, later symmetrical ones. The pain improves when you use affected joints. Also there is the morning stiffness of all joints for an hour or more. In osteoarthritis pain worsens in opposite way - over the day after use.

 

Erosion and destruction of the joint surfaces eventually leads to deformity. The fingers are typically deviated towards the little finger and make unnatural shapes: boutonniere deformity, swan neck deformity, Z-Thumb, "squared hand. Besides joints problems patients suffer anemia, gastrointestinal bleeding from drugs, splenomegaly (big spleen) and leucopenia (Felty's syndrome), the salivary and lacrimal (tear) glands inflammation (Sjogren's syndrome).

 

Dermatological marks of rheumatoid arthritis include subcutaneous (under skin) nodules mostly on elbows. Lungs may get fibrosis and pulmonary effusions. Blood vessel disorders lead to nail fold infarcts, neuropathies and nephropathies. Amyloidosis affects kidneys, heart and muscles. Heart also suffers form pericarditis and valve destruction. Dry eyes, sclera inflammation and softening may be found. Fissure and leaking of eye humor could happen. Neurological system is affected by neuritis and atlanto-axial subluxation. Eventually it may lead to compression of the spinal cord, paralysis and quadriplegia.

 

To diagnose rheumatoid arthritis American College of Rheumatologists developed the criteria.

* Morning stiffness more than an hour
* Arthritis and soft-tissue swelling of more than 3 joints
* Arthritis of hand
* Symmetric arthritis
* Specific subcutaneous nodules
* High rheumatoid factor
* Radiological changes showing joint erosion

 

At least four criteria are needed to establish the diagnosis.

Immunological and other studies help to confirm the diagnosis: rheumatoid factor, anti-citrullinated protein antibodies, erythrocyte sedimentation rate, C-reactive protein, blood count, renal function tests, liver enzymes, antinuclear antibody. HLA marker DR4 is an important risk factor.

Essential Oils Shown To Alleviate Depression & Pain In Arthritis Patients

I have been a fan of essential oils for over 10 years now. I have found them personally useful for alleviating many physical and emotional issues in that time. I also especially find the aromas of many of them extremely inspiring and uplifting.I have personally used a combination of Therapeutic Grade (an important concept in essential oils use) Frankincense and Lavender from Young Living Essential Oils for managing symptoms of depression as have some of my clients.

 

Here I report on a recent study done by Kim MJ, Nam ES, Paik SI.*** in Feb 2005 that employs a combination of lavender, marjoram, eucalyptus, rosemary, and peppermint in a carrier oil that looked at the effect of this combination on the symptoms of depression, pain and satisfaction in life in arthritis patients.

 

This study has some scientific significance as it was an unblinded study with a comparison control group. The sample consisted of 40 patients, enrolled in the Rheumatics Center, Kangnam St. Mary's Hospital, South Korea. It is important to keep in mind that there is no information about the methodology of the study provided in the abstract and that the power of the study could be increased through randomization of patients and blinding of treatment and assessment phases.

 

The study found that:

"Aromatherapy significantly decreased both the pain score and the depression score of the experimental group compared with the control group. However, aromatherapy didn't increase the feeling of satisfaction in life of the experimental group compared with the control group. "

 

Their final conclusion was:

"The result of this study clearly shows that aromatherapy has major effects on decreasing pain and depression levels. Based on our experiment's findings, we suggest that aromatherapy can be a useful nursing intervention for arthritis patients."

It is to be expected that the last parameter namely "satisfaction in life" would likely not respond as this is highly dependent on one's actual life situation and that this would in fact need to evolve or change.

 

It has been my experience that individuals who succumb to chronic debilitating illnesses become traumatized by the actual length of time they find themselves incapacitated.

 

This often imprints them with many fears of returning to a normal life. I have been helping such individuals overcome their fears with a new process (the Mind Resonance Process(TM) (MRP)) that is used to complement the essential oils.

The use of MRP in conjunction with Essential Oils is proving to be a useful strategy to mobilize such individuals back into a state of normalcy.

Have Arthritis? Exercise Can Help

Many people believe that you should not exercise if you have arthritis because it will damage your joints more. This couldn't be further from the truth though. Exercise can actually reduce your pain and stiffness.

 

Children with systemic juvenile arthritis can have several joints that are swollen and painful. There may also be a fever which lasts about two weeks. The fever is usually worse in the afternoon and evenings. Systemic juvenile arthritis accounts for about 20% of juvenile arthritis sufferers.Children with systemic juvenile arthritis may also have a pink rash that appears on the thighs and lower body.

 

If you have arthritis then it is very important that you get on a work out program. When you do exercises, it builds muscles around your joints and increases your flexibility. It can also give you more energy, help you to sleep better, keep you at a healthy weight, give you self confidence and decrease depression. Exercise improves your overall health! It can even reduce your risk of developing other health problems such as heart disease and osteoporosis.

 

For many people exercise is hard to do because they do not make it fun. It is a chore to do. It's really hard to be consistent about exercising when you are constantly dreading it. Try to pick an exercise routine that you enjoy, such as walking, dancing, doing yoga, swimming, or bicycling. Walking is one of the easiest to do and is an extremely efficient exercise. Even slow walking is beneficial because it will help the mechanisms in your body, put you in a better mood, help with any anxiety and give you more self-esteem. Regular walking will decrease your joint stiffness and swelling.

 

If you are in a lot of pain then you may want to start doing water exercises at first. This will reduce the stress on your knees, hips and spine. Flexibility exercises are great for arthritis. They are just stretches that help make it easier for you to move. If you have trouble doing your daily activities because of pain then you should definitely try this. Once you get used to doing these easier exercises then you can move on to more difficult things such as endurance and weight training. It is very important that you stretch before and after any exercising.

 

New studies have shown that many people actually get arthritis because of a lack of exercise. Things like television and computers are causing people to be less active than before. Because of this, more and more people are developing arthritis. Your body needs activity to be healthy. If you don't use the muscles and joints in your body regularly then they will degenerate. So if you don't have arthritis then the best way to prevent it is to exercise.

 

Before you start exercising you should talk to your doctor. They can help you decide what will be best for you. Physical therapists and occupational therapists can also be a great help to you. A physical therapist will show you the proper way to do exercises and walk you through them. An occupational therapist will help you by showing you how to do basic daily activities without hurting your joints.

There are over 100 different types of arthritis and exercise can help any of them. The best types of work outs to do are range of motion, strengthening and aerobic exercises. Just make sure you don't over exert yourself. If you exercise and have pain that lasts for more than an hour then you are exercising too much.
