Arthritis - Surgery?

If you suffer from severe osteoarthritis, surgery may be in store for you.

Surgery is no small choice, and doctors and the surgeons they work with are naturally reluctant to operate unless you meet criteria that they consider important.

Doctors call the replacement of an artificial joint gone bad a "revision."

Two of the key factors that all surgeons probably consider are the life-expectancies of the artificial joint and of you. Historically artificial joints last about 15 to 20 years. New technologies may extend this, but there's really no one offering a guarantee that this will be so. For the new technologies, the long-term data just isn't there yet. (That's why it's called "new.")

 

There seem to be two models that doctors and the surgeons they work with go by.

 

Model #1: Age and pain intensity.

In this model, the age is a critically important factor. If you're expected to live, on average, to age 75 or 80, and the joint is only going to last 15 years, surgeons using this model will be reluctant to operate until you're 65 years old.

 

The other parameter is pain intensity. No matter how old you are, if you need the replacement because the pain is just unbearable, many (most?) physicians will agree to operate.

 

Model #2: Add "life style"

In this model, the factors of the first model are still considered, but to them is added the question of whether you're sacrificing your life right now in order to have some kind of "ideal state" when you're 65.

This is clearly a subjective area for both the patient and the doctor, but in many cases, the evidence is so overwhelmingly clear that the call for a replacement of the joint is considered very reasonable.

 

And there are clear advantages to getting the artificial joint while you are younger.

 

* Generally, you're better able to withstand surgery when you're younger than when you're in your latter years.

 

* Assuming the operation and recovery go well, you'll have years of a better life that you can look back on when you're older - even if at that time you get sidelined because a revision isn't possible.

 

* Technology is improving all the time. By the time you need a revision (assuming that the newer materials do wear out in 15 to 20 years), the procedures may be in place to make revisions much easier to successfully perform.

 

Concluding Remarks

This article is a clarion call for arthritis patients to be advocates on their own behalf and to have a discussion of the effects that your arthritis is having on your life. If you are severely restricted in what you can do, if your family is suffering so as to compensate for your pain, and if you can imagine what it would be like for you to see your loved ones in pain and know that this is what they feel about you right now, then it may be past due for you to have this conversation.

 

And if your physician and surgeon will not take lifestyle as an important factor, seek out a second, third and fourth doctor who will.

What is Arthritis

 

Arthritis is all too well known by most of us as a source of discomfort and pain. There are different forms of arthritis, however, with distinctive symptoms and prognosis.Osteoarthritis(OA)/arthrosis is a disease mainly characterized by degenerative processes of the articular cartilage, but changes also involve the synovial membrane and the bone next to the cartilage. It is a gradual decay that most often affects the weight bearing joints (knees, hips and spinal joints) and the joints of the hand. A breakdown of the cartilage matrix leads to cracks and ulcers and a thinning of the cartilage with a loss of shock absorption.

 

Traditionally osteoarthritis has been connected to aging, obesity and repeated mechanical joint stress.

 

The underlying bone starts to thicken as a response to the increasing stress and bone spurs are formed. In the advanced phases of osteoarthritis, an inflammatory reaction in the synovial membrane can be seen. This severe degeneration causes pain, swelling, deformation and reduced range of motion. Predisposing factors ch as trauma or inherited abnormalities are also known to trigger degenerative changes and cause secondary osteoarthritis at even younger ages. New research is beginning to shed light on how osteoarthritis develops at the cellular and molecular levels. Arthritis and related conditions affect nearly 43 million Americans, or about one of every six people, making it one of the most prevalent diseases in the United States. By 2020, as the baby boom generation ages, an estimated 60 million Americans will be affected by arthritis.

 

Arthritis is one of the most pervasive diseases in the United States and is the leading cause of disability. According to the Centers for Disease Control and Prevention one out of every three Americans (an estimated 70 million people) is affected by one of the more than 100 types of arthritis. For most people arthritis pain and inflammation cannot be avoided as the body ages. In fact, most people over the age of 50 show some signs of arthritis. Joints naturally degenerate over time. Fortunately, arthritis can be managed through a combination of medication, exercise, rest, weight-management, nutrition, and, in some cases, surgery. Your doctor can tell if you have arthritis through blood tests and x-rays. He or she will then be able to help you decide on the best treatment for your case. Arthritis is a chronic disease that will be with you for a long time and possibly for the rest of your life. Your treatments will probably change over time and medication may be adjusted. Having a positive mental outlook and the support of family and friends will help you live with arthritis and be able to continue to perform your daily activities.

 

Signs and Symptoms of Arthritis
Arthritis is a term that includes a group of disorders that affect your joints and muscles. Arthritis symptoms include joint pain, inflammation and limited movement of joints. When a joint is inflamed it may be swollen, tender, warm to the touch or red. If you have experienced pain, stiffness, or swelling around a joint for more than two weeks, you should visit your physician. Only a physician can determine if you are suffering from symptoms of arthritis. You need to find out which type you have in order to start the best course of arthritis treatment. These arthritis symptoms may come up suddenly or slowly over time and may also include sleeplessness, fatigue, depression, and muscle aches. Many people will experience some difficulty functioning at home, at work or at play because of joint pain, stiffness and loss of motion regardless of the type of arthritis they have. Getting out of bed in the morning, buttoning buttons, writing, sewing, meal preparation, dressing, sleeping, walking, climbing stairs, arising from a chair or a toilet seat and attending to matters of personal hygiene may all be impaired to some degree by arthritis pain and joint stiffness. A lot of people find that impairment of mobility is more distressing to them than arthritis pain.

What You Should Know About Arthritis

 

Should the person experience some muscle pain which lasts more than 2 or 3 days, there is a strong chance that one has arthritis.

Arthritis is a common disease that affects millions of people worldwide. This can be felt in the person's joints, skin and organs inside the body. Should the person feel anything wrong, it is advisable to go straight to the doctor.

Doctors have discovered that there are over 100 types of arthritis.

Given the number, the doctor will not be able to know which one is affecting the person without an examination. There are 2 common forms of arthritis. The first is rheumatoid arthritis which is considered a chronic disease. There is inflammation in the joints caused by cartilage damage. Anyone who has this will suffer long term joint damage that will lead to chronic pain and disability. Pain is usually felt when waking up in the morning and will gradually disappear during the day.

 

Rheumatoid arthritis is a problem that will not go away. This happens in three stages. The first is swelling. The second is the rapid division and growth of cells. The third is when these cells release enzymes that will eat the bone causing the joint to lose shape until the person will not be able to move it anymore.

 

Since this is systemic disease, it can spread and affect other organs in the body. The best way to prevent is from happening is detecting it early to prevent the person from being disabled.

 

This can be treated with proper medication and therapy. There many drugs available that the patient can use. Some drugs offer pain relief to reduce the inflammation. Others can just do one function.

 

The second is called osteoarthritis. This happens more often than rheumatoid arthritis but unlike the first, there is no inflammation present. The cartilage in the joint is damaged and will eventually degenerate. Pain will slightly be felt when the person gets up but this will hurt later on during the day.

 

Osteoarthritis can either be primary or secondary. When it is primary, it is often associated with age. It is similar to a car where the parts have to be replaced due to wear and tear. Doctors consider this to be normal as people grow older.

 

The secondary type is often associated with something else that has caused this to happen. Some of these factors are an injury that took place, heredity, obesity and bone density.

 

Osteoarthritis can be treated with medication, exercise, weight control, joint protection, physical and occupational therapy. This is done to relieve the pain and slow the progression of the disease.

 

Both of these are caused by different things. The common thing between these 2 types is that joint pain can happen anywhere in the body.

 

Given the many medications available to treat this disease, the patient has to be aware of the side effects of each before choosing which one to use. The doctor should explain these to the person in order to make the right decision.

 

Arthritis and it's different forms will not go away. Since this is an insurable disease, the best thing that the doctor and others can do until a cure if found is to help the patient ease the pain the slow the growth of the disease.

The Futility of Medical Diagnosis - Case History of Arthritis

A man, who had been treated at a similar place, as I had gone to get rid of my arthritis, said this to Dr. Gross. "I have been to hospitals and specialists who took as much as a week and thousands of dollars to give me a through examination and I did not get well.

 

I came here and did not get an examination and got well. I just can not understand it."

That man had come to this spa a few weeks earlier. He was an important businessman. I tell you this so you know he had money for any type of treatment. He had developed arthritis. He had gone the rounds of the best care the medical profession could offer. After years of this he came to this spa.

 

He was looked over. He had swollen and stiffened joins and Dr. Gross spent about thirty minutes getting a complete story of his mode of living and of the abuses of which he had been subjected in the form of treatment. He was told he was going to bed for a complete bed rest and was going to give him all his meals out of a pitcher. Right, Just water.

 

Upon hearing this, the man asked, perplexed, "Aren't you going to examine me? He was told they had all the information they needed. He went to bed. After a few days he began to see improvement in his condition. He continued to improve. He had hobbled in on a cane and crutch. After 2 weeks he was a puzzled but happy man. He was in great health and no pain nor any inflammation in his previously affected joints.

 

It was explained to him that examinations are searches for symptoms, effects, results, end poisons, and not searches for CAUSE. If cause is known, it can be removed, whereupon the production of effect ceases. If cause is not known, palliation of symptoms is all that can be hoped for while the means of palliation produce complications of their own.His way of living was the cause of his arthritis and the treatment of changing his way of living was the results he was seeing.He was told the story of the physician who analyzed the teardrops of the woman who had been crying for days. He found the teardrops to be composed of water and saline particles, but he failed to find the cause of her crying.

 

A knowledge of the composition of the teardrops was of no service to him in his care of the crying woman.

Had he known or asked the cause of her crying, he might have been able to be of genuine service to her.

 

It was pointed out to the man that the causes of a disease are not found in its symptoms nor in the morbid changes in the fluids and tissues of the body that are part of the disease. Cause has to be sought for in other places

 

Removal of causes is necessary for health improvement.

Patients do not understand that when a urine test shows sugar in the urine, this does not reveal the cause of the glycosuria. Neither do they know the significance of the oft-repeated statement that the cause of diabetes is unknown.

 

It is easy to test the urine and discover the presence of sugar. It is almost equally easy to discover excess blood sugar by means of a blood test and neither test reveals the cause.

 

Without knowledge of cause all a physician can do in his care of a diabetic patient is palliate symptoms. The cause of diabetes are ignored, not understood, and not sought for so long as the physician and his patients are satisfied with the palliation.

 

A similar approach is made to arthritis. Once the elaborate exam is over and the diagnosis has been made the patient is given Aspirin, Gold salts, cortisone and numerous harmful other drugs to lessen inflammation and to enable the sufferer to move about, often with the aid of cane or crutch, while growing progressively worse as the arthritis spreads from joint to joint.

 

The above example was from an examination taken in the 1920's. Now almost 85 years later nothing much has changed, except the medical field has more dangerous drugs to give.

 

The medical profession is hopelessly bogged down in a morass of diagnostic techniques, and therapeutic failures from which it is unable to extricate itself.

 

Like my 7 day stay in the hospital. Test after test for why I had been dizzy for a short period. I was no longer dizzy but did they care? The first doctor to see me said it was from an infected ear and did nothing for that but gave me blood pressure medicine due to my blood pressure being higher while in the hospital.

 

It made me ill and I stopped it. All those test and no treatment. Said I had hyper-tension. Up to my stay in the hospital I had been 120/80 for 77 years.

 

All degenerative diseases are reversible if caught early enough. Even desperate cases are reversed if they follow the right treatment. What is this treatment?

 

Fasting. That's right. No food just water. The problem is that most people cannot leave their home for 2 weeks or more for this treatment. But I now have the means for you to do this at home.

Natural Relief For Arthritis And Joint Pain?

 

The years of teaching high impact aerobics are catching up with me. Sure it's been great for the ticker, but the knees on the other hand are showing signs of age and making icky noises. What my Physical Therapist friends call "creep" and "crepitus" (which is just nice technical way of saying "grinding") had become loud enough to be heard by the naked ear! At the rate I was going - I was sure I'd need a knee replacement by 40. Of course this just won't do. So I've taken to doing leg extensions regularly and... taking glucosamine.

 

Glucosamine - What's That?

"Glucosamine" is a natural constituent of cartilage which has been shown to stimulate the production of connective tissue! In more technical terms, glucosamine is considered an "amino sugar." An amino sugar is the component of a carbohydrate which does not contribute to the body's energy - instead it gets incorporated into body tissues, forming such structures as tendons, ligaments, bones, skin, nails, eyes and heart valves.

 

What is Glucosamine Used For?

Glucosamine supplements have been used most recently (according to the research I consulted) for everything from joint pain to connective tissue repair. It's best know for its contribution to tendon and ligament support, for building joint cartilage as well as reducing destruction of cartilage. It has also shown promise in reducing inflammation due to asthma and bursitis, lessen the incidence of food allergies, tendonitis and skin problems! As if that isn't enough - what I find most exciting about glucosamine is its use in relief of osteoarthritis and rheumatoid arthritis.

 

How Glucosamine Works

It's fairly basic. We produce less glucosamine as we age, therefore our cartilage has difficulty retaining water. Depletion of this fluid, which ads "cushion" to the joints and other tissues, can create everything from joint pain to arthritis. Glucosamine (together with its counterpart chondroitin - another naturally occurring substance) - helps to counteract this water loss as well as restore cartilage.

Side Effects

 

So What's the bad news? I always want to know. Here's what I found: Since these substances are naturally occurring - they work on the body differently than synthetic drugs (think Prednisone) or anti-inflam's like Naproxen. This said - it takes a while to notice signs of joint help - more than likely it could take up to a year of regular supplementation before the benefits take hold. Once they do, however, 50% of all regular users have reported definite decreases in symptoms of joint pain - I will let you know! The only other downside I could find to the glucosamine/chondroitin complex users were nausea and heartburn - although they are rare and counteracted by eating prior to supplementation.

How To?

 

As I've just stated, always take the glucosamine/chondroitin complex with meals to avoid side effects. I take one, 500 mg tablet once a day. Dr. Mindell (I am a big fan of his work: see below) recommends taking just 1-3 500 mg. tabs daily for three weeks (to get a boost), then take one, 500 mg. tab daily.

As always, please check with your healthcare provider before supplementation - discuss with him/her any medications you are taking to avoid interactions. Although herbs and other naturally occurring substances are considered "food" (simply because they can't be patented and trademarked thus cannot come under scrutiny of the FDA) they are still very strong substances in the body and should not be taken lightly! Psoriasis Skin Inflammation & Psoriatic Arthritis

A person affected might first mistake it for fungal infection, a disease that also causes white patches to appear on the skin. A big difference between the two is the inflammation of the affected area and the silvery white scale that covers it. These patches are called "lesions." The coverage area of these lesions can range from minimal to widespread depending on the person affected. For most people, though, psoriasis tends to be mild. Unlike fungal infection also, psoriasis is not contagious. Contrary misconceptions, it cannot be passed on to another person by skin contact.

 

And revisions are sometimes more difficult than the original operation. This also forms one more cause for reluctance in treating younger patients.

 

Psoriasis can develop on any part of the body but it commonly appears on the scalp, knees, elbows, and torso. The appearance of lesions is often symmetrical. That's why when a lesion appears on the right side of the body, the person affected should expect one to appear on the left also.

 

Psoriasis rarely appears on children. It usually develops at the age of 15 to 35 and occurs equally both on men and women no matter what race.

 

Occurrence of psoriasis can range from mild to moderate to severe. When three to ten percent of the body is affected, it is considered a moderate case, but when more than ten percent is affected, it is already considered severe. Whatever the case of psoriasis a person has, though, it always affects his or her quality of living because of the health complications it entails. The skin, which is its primary target, can appear to be discolored if severe psoriasis occurs.

 

Another health complication that is linked with psoriasis is psoriatic arthritis. It may develop anytime but, mostly, it occurs in the adults. Although having psoriasis does not necessarily mean that psoriatic arthritis will follow.
