Juvenile Arthritis (part 1) Why Does Arthritis Affect Children?

There are three main types of Juvenile arthritis (or Juvenile Chronic Arthritis).

Juvenile arthritis (JRA) is not a disease of wear & tear. JRA can be genetic disease, affecting children whose immune system is prone to viral attack. In fact Juvenile Arthritis is actually a group of diseases which affect children

 

The most common form of juvenile arthritis is Pauciarticular Juvenile Arthritis. This accounts for more than half the cases of arthritis in children. This disease seems to affect girls more than boys and usually affects the larger joints knees, ankles, elbows, and hips, these joints can become swollen and painful. It affects younger children, mainly under the age of four.Pauciarticular juvenile arthritis can also affect the eyes and should be watched carefully by an eye specialist as significant eye damage can occur as a result.

 

Watch your child carefully if he or she develops a limp. This can be the first sign your child has developed Pauciarticular juvenile arthritis. An accurate diagnosis unfortunately can't be given much before six weeks, however medical attention should be sought as soon as there is any suspicion your child may be affected. This is most important to rule out other diseases with similar symptoms such as rheumatic fever.

 

The treatments typically offered are those that reduce pain and inflammation for example aspirin. Although aspirin is better tolerated by children than it is by adults, it isn't without side effects.Pauciarticular juvenile arthritis can respond well to natural arthritis treatments too. Try Omega 3 oils (found in oily fish or in capsule form) and see if this can lower the dose of your child's medication while still maintaining their comfort level. A good diet with all the main food groups will help too.

 

Exercise is vital to prevent stiffness and maintain your child's health. Exercise which doesn't further irritate the joints is the best. Swimming is excellent for this, stretching exercises are great too. If the whole family joins in it can be fun! Exercise releases endorphins into the body, endorphins are a natural antidote to pain and depression.Juvenile arthritis is unique in that a child's joints are still growing and developing, a large part of treatment is making sure this happens as normally as possible and to monitor this growth.And now for some good news at last. Many cases of Pauciarticular juvenile arthritis can spontaneously disappear as you child grows older. This has to be the best outcome of all!

Natural Relief For Arthritis And Joint Pain?

 

The years of teaching high impact aerobics are catching up with me. Sure it's been great for the ticker, but the knees on the other hand are showing signs of age and making icky noises. What my Physical Therapist friends call "creep" and "crepitus" (which is just nice technical way of saying "grinding") had become loud enough to be heard by the naked ear! At the rate I was going - I was sure I'd need a knee replacement by 40. Of course this just won't do. So I've taken to doing leg extensions regularly and… taking glucosamine.

 

"Glucosamine" is a natural constituent of cartilage which has been shown to stimulate the production of connective tissue! In more technical terms, glucosamine is considered an "amino sugar."

 

Glucosamine - What's That?

An amino sugar is the component of a carbohydrate which does not contribute to the body's energy - instead it gets incorporated into body tissues, forming such structures as tendons, ligaments, bones, skin, nails, eyes and heart valves.

 

What is Glucosamine Used For?

Glucosamine supplements have been used most recently (according to the research I consulted) for everything from joint pain to connective tissue repair. It's best know for its contribution to tendon and ligament support, for building joint cartilage as well as reducing destruction of cartilage. It has also shown promise in reducing inflammation due to asthma and bursitis, lessen the incidence of food allergies, tendonitis and skin problems! As if that isn't enough - what I find most exciting about glucosamine is its use in relief of osteoarthritis and rheumatoid arthritis.

 

How Glucosamine Works

It's fairly basic. We produce less glucosamine as we age, therefore our cartilage has difficulty retaining water. Depletion of this fluid, which ads "cushion" to the joints and other tissues, can create everything from joint pain to arthritis. Glucosamine (together with its counterpart chondroitin - another naturally occurring substance) - helps to counteract this water loss as well as restore cartilage.

 

Side Effects

So What's the bad news? I always want to know. Here's what I found: Since these substances are naturally occurring - they work on the body differently than synthetic drugs (think Prednisone) or anti-inflam's like Naproxen. This said - it takes a while to notice signs of joint help - more than likely it could take up to a year of regular supplementation before the benefits take hold. Once they do, however, 50% of all regular users have reported definite decreases in symptoms of joint pain - I will let you know! The only other downside I could find to the glucosamine/chondroitin complex users were nausea and heartburn - although they are rare and counteracted by eating prior to supplementation.

 

How To?

As I've just stated, always take the glucosamine/chondroitin complex with meals to avoid side effects. I take one, 500 mg tablet once a day. Dr. Mindell (I am a big fan of his work: see below) recommends taking just 1-3 500 mg. tabs daily for three weeks (to get a boost), then take one, 500 mg. tab daily.

Arthritis Basics

Arthritis affects people in a variety of ways. Joints may be stiff and creak. For others who suffer from arthritis, joints might crack suddenly, like knees upon standing. For many arthritis sufferers, pain occurs, like when trying to open a jar.

Arthritis actually means “joint inflammation” and has over 100 related conditions or forms of the disease.

What’s it all about? Let’s take a look at the basics of arthritis and learn more.

If arthritis is left untreated, it can advance, resulting in joint damage that cannot be reversed. Therefore, early detection of arthritis and arthritis treatment are important.

 

The two most common types of arthritis are osteoarthritis (OA) and rheumatoid arthritis (RA). Although both type of arthritis have similar symptoms, both happen for different reasons. When joints are overused and misused, the results can be osteoarthritis (OA). What happens is that the cushioning cartilage that protects the joint breaks down, resulting in the bones rubbing together. This generally happens in the knees, but can be found in the hips, spine and hands as well. And, only in the later stages of osteoarthritis will a person most often feel pain, after quite a bit of cartilage is lost.

 

The second type, rheumatoid arthritis (RA), refers to the body’s immune system attacking joint tissue. Still not fully understood in the medical community, rheumatoid arthritis most often starts in a person’s hands, wrists and feet. Then rheumatoid arthritis advances to shoulders, elbows and hips.Similar symptoms include pain, stiffness, fatigue, weakness, slight fever, and inflamed tissue lumps under the skin. And both osteoarthritis and rheumatoid arthritis generally develop symmetrically, i.e. affecting the same joints on both the left and right sides of the body.

 

A difference in osteoarthritis and rheumatoid arthritis to note is with swelling. With rheumatoid arthritis, people report “soft and squishy” swelling. While with osteoarthritis, people report “hard and bony” swelling.Another difference between the two types of arthritis is that a person is more likely to develop rheumatoid arthritis if a sibling or parent had it. While a person with a history of joint damage, either an injury or chronic strain, runs a higher risk for developing osteoarthritis.Arthritis can affect any age group. However, while there is no specific age for arthritis sufferers, arthritis seems to focus on those over 45 years of age.

 

And, while neither gender is immune, a reported 74 percent of osteoarthritis cases (or just over 15 million) occur with women and a slightly lower percentage of rheumatoid arthritis cases occur with women.Furthermore, your health, particularly your weight, plays a role in the development of arthritis. People with excess weight tend to develop osteoarthritis, especially in the knees when reaching over 45 years of age. However, losing weight can turn the odds around almost by half. Regular activity combined with exercise also reduces risk of arthritis, strengthening joint muscles and reducing joint wear.

 

Although there are no cure-alls for arthritis, there are a variety of pain relief treatment strategies. Aside from medications, remedies, replacement alternatives and other helpful treatment options, the four main arthritis relief aids are gentle exercise, good nutrition, a positive attitude, and rest. Education also plays a huge role to dispel “old wives tales” and myths that “nothing can be done about arthritis.”

 

If you suspect you may have arthritis, it is advisable to seek medical advice. The sooner you detect the arthritis and the type of arthritis the better off you’ll be. Furthermore, your symptoms may not be caused by arthritis, but may be caused by something else like a virus or tendonitis or other similar problem that could potentially worsen if left untreated.

Doctor, Should I Take Humira for My Rheumatoid Arthritis?

 

TNF alpha is a cytokine, a unique protein that is produced during the inflammatory response. TNF alpha is not only the result of inflammation, it is also a substance that promotes the inflammation. Increased levels of TNF are found in several inflammatory conditions including rheumatoid arthritis, psoriatic arthritis, and ankylosing spondylitis. Drugs that block tumor necrosis factor (TNF) have been found to be particularly effective for the treatment of these serious forms of inflammatory arthritis.Adalimumab (Humira) is constructed from a fully human monoclonal antibody. It binds to TNF alpha, preventing it from activating TNF receptors.

 

It acts like a barrier to the interaction between TNF alpha and receptors for TNF alpha on immune cells. This prevents TNF alpha from perpetuating inflammation and the pain, tenderness and swelling of joints in patients with different types of arthritis.Humira reduces the signs and symptoms of rheumatoid arthritis, the arthritis of psoriasis, and ankylosing spondylitis. It also prevents the progression of joint destruction in patients with rheumatoid arthritis and the arthritis of psoriasis.Humira can be used alone or in combination with methotrexate. The drug has been approved for use in rheumatoid arthritis, psoriatic arthritis, and ankylosing spondylitis.

 

It comes in a 40 mg prefilled syringe and is administered subcutaneously every two weeks. The syringe must be kept refrigerated.

Serious infections, including tuberculosis, have occurred in patients receiving Humira. In some cases, these infections have been fatal. Before starting the drug, a patient should be tested for TB. Any medication prescribed for the treatment of TB should start before beginning Humira and should be continued until the full course of medication is completed.

 

Since Humira suppresses important parts of the immune system, a patient should not receive vaccines containing live virus.

Having an infection could put a patient at risk for serious side effects from Humira.

TNF-blocking agents have been associated with reactivation of hepatitis B. Some cases have been fatal.

Rare cases of demyelinating syndromes such as multiple sclerosis have been reported.

Anti-TNF drugs should be used with caution in patients with congestive heart failure.

 

Combining Humira and Kineret (anakinra) is not recommended

Humira is contraindicated in pregnancy as well as for use in nursing mothers.

 

There have been rare cases of severe allergic reactions after taking Humira. Lymphoma and pancytopenia (low blood counts) have also been rarely reported in patients taking anti-TNF therapy.The most common side-effects are injection site reactions, upper respiratory tract infections, headache, and nausea.

"My Doctor Wants Me to Take Enbrel For My Rheumatoid Arthritis-What Can You Tell me About it?

It is not only the product of inflammation; it also helps perpetuate and promote inflammation. Increased levels of TNF are found in several inflammatory conditions including rheumatoid arthritis, psoriatic arthritis, and ankylosing spondylitis. Drugs that block tumor necrosis factor (TNF) have been found to be particularly effective for the treatment of these serious forms of inflammatory arthritis.

 

The first anti-TNF drug approved for such use was etanercept (Enbrel).

Etanercept is a synthetic (man-made) protein that binds to TNF alpha. It acts like a sponge to remove most of the TNF alpha molecules from the joints and blood. This prevents TNF alpha from perpetuating inflammation and the pain, tenderness and swelling of joints in patients with different types of arthritis.Etanercept reduces the signs and symptoms of rheumatoid arthritis, the arthritis of psoriasis, and ankylosing spondylitis. It also prevents the progression of joint destruction in patients with rheumatoid arthritis and the arthritis of psoriasis.

 

Etanercept is usually used in combination with methotrexate in patients who do not respond adequately to methotrexate alone.

Etanercept comes in two different preparations. The first is a powder that must be reconstituted (mixed) with a diluent. This comes as a 25 mg dose. The second is as a premixed syringe containing 50 mgs of etanercept. Etanercept must be refrigerated. The drug is given as a subcutaneous injection. Dosing will vary according to disease severity and body size.Because etanercept reduces the immune response, it should not be administered with live vaccines. Etanercept is not recommended for use in pregnant or nursing women.The most common side effects are mild to moderate itching, pain, swelling and redness at the site of injection. Headache, dizziness, nose and throat irritation also occur.

 

TNF alpha has an important role in the response of the immune system to infections. Blocking the action of TNF alpha with etanercept may worsen or increase the occurrence of infections, and patients with serious infections should not receive etanercept. Etanercept should be discontinued if a patient develops a serious infection. It should not be given to patients with active infections or who have an allergy to its components. Etanercept should be used with caution in patients prone to infection, such as those with poorly controlled diabetes.

 

Since etanercept has entered the market, there have been reports of multiple sclerosis, myelitis, optic neuritis in patients using the drug. Etanercept is not recommended for persons with preexisting disease of the central nervous system (brain and/or spinal cord) or for those with multiple sclerosis, myelitis, or optic neuritis. Rare cases of potentially serious low blood counts (pancytopenia) have been reported in patients using etanercept.Monitoring should be done according to guidelines set by the rheumatologist.

A Canadian class-action lawsuit has been launched against Pfizer Inc. alleging that its arthritis drug Celebrex caused cardiovascular side effects, the law firm launching the suit said Friday."Merchant Law Group included Pfizer's Bextra in its suit and also named Boehringer Ingelheim, Germany's largest pharmaceutical firm, for its Mobicox, in the same class of drugs.

Doctor, Help Me - How Do I Find Arthritis Pain Relief?

The first and most important activity a doctor must do when the patient comes in is to make an accurate diagnosis. This is done with a careful history, physical exam, laboratory tests, and imaging procedures.

 

Pauciarticular, Polyarticular and Systemic. In this article we'll concentrate on Pauciarticular juvenile arthritis.

 

In the history, we ask questions such as:
• How long have you had the symptoms?
• What joints are involved?
• Is it symmetrical? One side like the other
• Is there a family history?
• Are there other symptoms?

On physical exam, we examine a patient from the top of the head to the bottoms of the feet! We look at the skin, eyes, ears, nose, throat, internal organs, and finally the joints

 

The goals of treatment are straightforward. They are:
• Relieve pain/inflammation
• Enhance quality of life
• Slow disease progression
• Control co-morbidity (associated diseases such as high blood pressure, diabetes, etc.)
• Minimize risks of therapy

We first start with non-medicine treatment:
• Social support: make sure the patient’s family and friends understand the problem
• Education: make sure the patient understands all the things they must do themselves to get better
• Weight-loss: many patients with low back pain, knee pain, and hip pain are overweight. All the medicines in the world aren’t going to help until weight is corrected.
• Assistive devices: splints, braces, walkers, canes, etc. all may help.
• Thermal modalities: ice or moist heat depending on the situation
• Exercise: non impact as well as stretching and strengthening play a role.
• Modification of lifestyle: sometimes habits need to change and routines need to be altered.

Medicines:
• Analgesics: These help reduce pain. They don’t block inflammation. Analgesics may be habit-forming or addictive. They offer the potential for side effects as well. Examples: Tylenol, Ultram, Darvocet, Percodan.
• Anti-inflammatory medicines: These block inflammation and help with pain. There is the potential for side-effects including the liver, kidneys, and cardiovascular systems. Examples: Naprosyn, Motrin, Celebrex.
• Disease-modifying drugs: these drugs slow down the progression of arthritis. They are used in conjunction with analgesics and anti-inflammatory medicines. Wxamples: hydroxychloroquine (Plaquenil), methotrexate, azathioprine (Imuran).
• Biologic therapies: these are lasers that target the immune abnormalities found in many forms of arthritis. Examples: Enbrel, Humira, Remicade.

Specifically for arthritis related pain we also use medicines such as GABA stimulators such as gabapentin (neurontin) and pregabalin (Lyrica).

 

Lidoderm patches also help as do topical agents such as Myorx.

Sometimes injections of different types will be needed. These injections may be combinations of local anesthetics and glucocorticoid or they may consist of materials such as Botox. We have used the latter quite successfully in patients with neck and low back problems.
